Every recommendation whether related to medication timing, wound care, activity limitations, or follow-up visits is based on the patients records.
 
The app’s front-end is built for patient engagement and ease of use, providing intuitive tools to log recovery metrics, track medication adherence, and communicate through an integrated chat interface. Patients can ask questions about their recovery, medications, or follow-up instructions and receive responses generated directly from their discharge documentation. On the back end LLM references the patient’s specific clinical data via the RAG index, ensuring responses are contextual and medically aligned.
 
Reliability and clinical governance are reinforced through a Human-in-the-Loop (HITL) framework. When the AI detects ambiguous situations, missing data, or potential red-flag symptoms, it automatically escalates the case to a designated clinician for rapid review. If a patient asks a question outside the scope of the discharge document, the system responds transparently and directs them to contact their care team instead of improvising.
